How do I fill out Form DD 1056?
Since this form will be submitted with a passport application, the applicant must identify the date a passport will be required by the applicant. In second box of the form the applicant should provide the service or group the individual is in that qualifies them for the fee waiver. Applicant’s personal information should be provided as well. If the applicant requires a sponsor for passport application, the sponsor must put their information in boxes 6, 7 and 8. Boxes from 11 through 15 require the applicant to list their travel plans the passport will be used for. Applicants must identify their destination, special assignments, agency to forward passport to, and time of arrival.
An authorized official must fill out the box 16 indicating approval of the DD Form 1056 for the applicant. Any additional comments may be entered in box 17 by the supervising authority.
